Most falls in elderly parents really are fine. But here's the honest truth we've learned from 30 years of medicine: some of the more serious effects of a fall don't show up immediately. They can take a day or two to appear. Which means "seems okay right now" isn't always the same as "is okay."

This isn't a guide to frighten you. It's a guide to help you check properly, so you can either truly relax or get help in time — whichever the situation calls for.

Why falls in older adults need more attention

Older bodies are different. Bones may be more brittle. Skin tears more easily. Reactions to injury can be slower and quieter. Things like:

  • A small head bump that seems harmless can lead to slow bleeding inside the skull, with symptoms appearing 12 to 72 hours later
  • A hip injury can be a hairline fracture even when the person can still stand
  • Hidden internal injuries can stay quiet for hours

This is why a quick check after any fall — even a "small" one — is genuinely one of the kindest things you can do.

What to gently check right after a fall

In the first few minutes, calmly look for:

  • Pain anywhere — especially hip, back, shoulder, wrist, or head
  • Difficulty standing, walking, or putting weight on a leg
  • Visible bruising, swelling, or deformity
  • A bump or cut on the head
  • Confusion, drowsiness, or unusual quietness
  • Loss of consciousness, even for a few seconds

If any of these are present, don't wait. Call a doctor right away.

Signs to watch for over the next 48 hours

This is the part most families don't know about. Even if your parent seems perfectly fine right after the fall, gently keep an eye on the next two days for:

  • Worsening headache or dizziness — especially after a head bump
  • Vomiting or nausea that wasn't there before
  • Drowsiness, confusion, or trouble recognising familiar people
  • Slurred speech or weakness on one side
  • New pain that wasn't there immediately — sometimes hip or back pain emerges as the initial shock wears off
  • Difficulty walking, limping, or refusing to put weight on a leg
  • Increased bruising or swelling
  • Unusual tiredness or "just not themselves"

Any of these in the days following a fall is reason enough to call a doctor.

When to go to hospital immediately

Don't wait for a home visit if you see:

  • Unconsciousness or inability to wake them
  • A seizure
  • Vomiting along with confusion after a head injury
  • Severe pain that prevents any movement
  • Visible deformity of a limb (suggesting a serious fracture)
  • Heavy or uncontrolled bleeding
  • Sudden weakness, slurred speech, or facial drooping

In these situations, call 108 or get to the nearest hospital straight away.
